NEW PATIENT PAPERWORK

 

Thank you for considering us as your Olympia Chiropractor. We are honored to work with you, and want to make your first visit as pleasant as possible.

 

Please arrive at least 20 minutes ahead of your scheduled time so that you can fill out an Intake Form and other required paperwork. Please bring a photo ID and your insurance information with you.

 

If you would like to save some time, you may download the forms by clicking the links that correspond to your situation. Print them and fill them out at home, then just bring them with you for your first appointment and arrive 5 minutes before your appointment time. You may also find it easier to fill out the necessary paper work online with the "Book Online Now" option.

If you were recently in an automobile accident please fill out the new patient form as well as a Motor vehicle accident (MVA) form and bring it with you.
